Sub: Treasuries and Accounts Department –Medical Reimbursement claims of dependent
family members – Furnish a copy of  family members entry made in the service Register–
Instructions – issued -Reg.

The attention of the DT&AO’s in the state is invited to the subject cited, it is
instructed that, while forwarding medical reimbursement claims of dependent family
members, they are requested to furnish copy of 1st page of SR along with the family
members declaration for in-service employees & copy of the PPO with attestation 
for the retired employees in addition to the dependent certificate attested by the
Treasury Officer.
