No.2601846/Prog.II/A2/2024-5 of School Education (Prog.II) Department, Dt:01.07.2025 Order: All the District Educational Officers and Additional Project Coordinators are informed that vide reference 6th cited, the Government issued memo for conducting the Mega Parent Teacher Meeting 2.O across the Government, Private Aided and Private Unaided schools and Junior Colleges in the state on 10th July 2025. It is decided for documentation of the conduct of Mega Parent Teacher Meeting 2. O in all the Government, Government Aided and Private Unaided Management Schools across the State. In view of the above, Witness Statement template format and SoP are communicated with these proceedings as Annexure-1 (Witness Statement template) and Annexure-II( SoP for Witnesses). # SoP for Witnesses The Samagra Shiksha Andhra Pradesh is collecting valid data of the Mega PTM for the effective planning of the State’s education transformation. All the District Educational Officers and Additional Project Coordinators are requested to inform all concerned school officials to urgently identify an independent witness outside the Ministry of Education and the parent community to observe the event and provide their valued observation of the following details of the MEGA PTM to be conducted on July 10, 2025. Below are the guidelines for documentation of Mega PTM. We request you to identify independent witnesses to your school for the event before 5th July 2025 9PM and register their details in the LEAP application, PTM Module **Documentation for Mega PTM:** * Every school HM should draft one independent witness (Observer) for recording the data occurring at the event * The independent witness should not be from the Education Department, NGO or should not be any parent of the same school student. * Local attorneys, Government officials, accountants, and police officers are some examples of valid witnesses. * HMs should register the details of the identified independent witness in the “ LEAP application > PTM module > Witness Registration tab” before end of 5th July 2025 9:00PM * A photo of the ID card of the independent witness of the organization they are working. It should also be uploaded in the application **Duties/Role of the Independent Witness on the Day of the event (10th June, 2025):** The independent witness is requested with observing the entire event and providing a detailed account of precisely what took place. The independent witness should be full time available at the venue/school of allotment on the day of event during the event * The Witness must be present at the designated school by 8:00 am and must remain present till the end of the event on the MEGA PTM day, 10th July 2025 * He / she must visit all the classroom and count the number of parents/guardians that attended and have a manual count * He / She must visit all classrooms and count the number of teachers present in the school on the day of the event * At the end of the event, all parent and teacher attendance should be verified by the witness by using his login provided by the authorities to login into the LEAP App and approve the data.
